TO RECEIVE YOUR PET ID TAG VIA MAIL:

1. Purchase any (2) TWO ARM & HAMMER® Clumping Cat Litters before 12/31/12 or use your special voucher.

2. Circle the cat litter and corresponding prices on your original cash register receipt(s). (Not required when using a special voucher.)

3. Follow instructions for completing your order form on the right, and press the Register and Print My Order Form button. Your order form will be displayed.

4. Print the order form on your printer using the "Print my Order Form" button.

5. Mail the completed Order Form showing your order number, and include your sales receipt(s) or your special voucher to:
